At West County Health Centers, we believe that everyone deserves access to high-quality, comprehensive primary care.

Our team of health care providers and support staff work collaboratively with our Behavioral Health, Wellness, Psychiatry, Perinatal, Youth, and education departments to provide comprehensive, team-based care.  

Our primary care team is also available to support coordination of care for patients requiring specialty care, certain chronic medical conditions, for assistance with care transitions from the hospital or skilled nursing facilities, and with mental health or other service providers.

West County Health Centers has a medical provider on call 24 hours a day, 7 days a week. If you need to speak with the provider on call, please call us.
