West County Health Centers’ Primary and Preventative Services proudly offers comprehensive and affirming services to all trans, non-binary, and other gender-diverse individuals.  Our unwavering commitment to all members of our community, including our gender-diverse community, includes a wide range of medical, behavioral, and mental health support.

Services

This service provides full-spectrum, comprehensive health care that respects and aligns with the patient’s gender identity. This includes preventative, reproductive, and sexual health, and chronic health care management tailored to the specific health needs of transgender and gender diverse individuals of all ages.

This includes psychiatric care, individual counseling services, surgical letters of support, and group medical visits tailored for youth and adults. These services aim to support mental well-being, address needs related to gender identity and foster a supportive community environment.

Gender affirming care for gender diverse youth is managed through family primary care teams.  Patients and families work together with medical and behavioral health care providers to develop a treatment plan centered on your gender goals, incorporating age, medical history, development, and mental health conditions.  Understanding your goals and concerns allows us to support you and your family.

Please contact us for further information related to specific care coordination and treatment for hormone therapy, letters of support, surgical referrals, and pre-operative and post-operative care.

This includes confidential testing, expert counseling, and treatment for sexually transmitted infections (STIs) and human immunodeficiency virus (HIV), delivered in a non-judgmental, inclusive manner. Access to Post-Exposure Prophylaxis (PEP) and Pre-Exposure Prophylaxis (PrEP) to prevent HIV infection, along with counseling on how to use these preventive medications effectively.

Billing and Administrative Information

Insurance Billing: We aim to address you accurately and respectfully. While we will bill under your insurance name and gender, we recognize the importance of acknowledging and using your chosen name and pronouns. 

Please provide us with both your chosen name and pronouns and your insurance or legal name and gender if they are not the same on your insurance billing information. This enables us to serve you with respect and understanding, ensuring your needs are met appropriately.

Schedule an Appointment: New patients can call any WCHC Primary Care site number, or our general number at 707-824-3391, and let the scheduler know that you want to establish care with a gender affirming medical provider as part of the Gender Services Program. You should typically be able to access a new patient appointment within 1 month.  If you have an outside primary care home and want to consult us only, please request a referral or RAF from your primary care provider first and then we can schedule you.

Services are provided at all WCHC Primary Care sites from Monday through Friday.
